Overview

We are seeking a compassionate and dedicated Registered Nurse (Home Health) to join our client's growing team in Reed City, Michigan. This position is focused on providing high-quality, patient-centered nursing care to individuals in their homes, promoting independence, recovery, and comfort. The ideal candidate is skilled in clinical assessment, adaptable in dynamic environments, and passionate about improving patient outcomes.

Key Responsibilities

Perform comprehensive patient assessments and deliver skilled nursing care in home settings

Develop, implement, and evaluate individualized care plans in collaboration with physicians and the interdisciplinary team

Provide patient and family education on disease management, medications, and home care procedures

Monitor clinical progress, adjust treatment plans as needed, and document all care accurately and timely

Communicate effectively with physicians, case managers, and team members to ensure coordinated care delivery

Support patients in achieving optimal health outcomes and maintaining quality of life at home

Schedule and Shift Details

Monday through Friday, 8 : 00am 5 : 00pm

One weekend per month (on-call)

Two holidays per year

Flexible schedule with minimal weekend requirements

This is a Pay Per Visit position

Travel

Local travel required throughout the Reed City area (travel reimbursement provided)

Qualifications

Required

Associate degree or higher in Nursing

Active Registered Nurse (RN) license in Michigan

Minimum of 1 year of professional RN experience (home health preferred)

Current BLS or ACLS certification through the American Heart Association

Valid driver's license, reliable transportation, and current auto insurance

Strong organizational, communication, and assessment skills

Preferred

Previous home health or community-based nursing experience

Experience with OASIS documentation and home health compliance standards
