Position Summary

As a Hospice Home Health Aide, you will function under the supervision of the RN Case Manager and Hospice Aide Manager as part of an interdisciplinary team. The role of the Hospice Aide is to assist terminally-ill patients with their personal care needs, including: personal hygiene care such as bathing/showering, incontinence care, dressing, skin/nail/oral care; assisting with ambulation, transfers, and repositioning; obtaining vital signs; light housekeeping; and other related support services essential to the client's health and well-being. The goal is to assist each patient in achieving the highest level of comfort and quality of life, consistent with their health status, wherever the patient lives. This is a full-time, Monday-Friday position.

• Provides personal cares as detailed in the care plan
• Displays an attitude of courtesy and respect for patients, families, staff, community partners, and referral sources
• Seeks and accepts feedback regarding the performance of job duties
• Is aware of patient confidentiality and adheres to HIPAA regulations
• Displays an understanding of vulnerable adult issues
• Displays an understanding of hospice patients' rights
• Completes all required documentation in timely manner
• Demonstrates a desire to work with the terminally ill

Qualifications

• Listed on MN nursing assistant registry with current Nursing Assistant Certification (CNA)
• 1-year experience as a home health aide or nursing assistant
• Current valid MN Driver's License and daily access to reliable, insured vehicle
• Ability to use and/or learn electronic charting on tablet and/or mobile phone
• Demonstrates a desire to work with the terminally ill
• Ability to demonstrate positive and respectful communication/interaction skills
