Clearwater Assisted Living, LLC is a rapidly growing assisted living organization with core competencies in assisted and transitional care throughout Wisconsin. Our primary staff are Direct Support Professionals who assist our clients in-home offering assistive services.

General Role Description :

We're currently looking for a Direct Support Professional to provide personal care assistance for our clients. The ideal candidate should be independent, empathetic and thorough on the job with a strong commitment to helping others. This is a part time position offering the hours of 20 - 25 per week.

Responsibilities :

• Client appointments, client cancellations or client updates that should be noted or addressed
• Consistently meet the expectations of completing client logs daily and reporting accurate mileage logs
• Follow the Individual Care Plan (ICP) for each client
• Uphold a high standard of care by showing up to work each day on-time with a positive attitude and integrity to work with your clients in a diligent manner
• Follow our scope of services and provide assistance as needed for encouraging and reminding clients to complete activities of daily living
