Job Opportunity : Lead MDS Coordinator, RN

Facility : Charlotte Bay Rehab and Care Center

Location : Port Charlotte, Florida

Position Type : Full-Time, 8 : 00 AM – 4 : 00 PM

Salary : $42–$46 per hour, depending on experience

Responsibilities :

Quality Care Oversight : Ensure the delivery of high-quality care to our long-term care residents.

Comprehensive Assessment : Conduct thorough initial interviews and assessments for new residents.

Care Planning : Develop individualized care plans focused on both short-term and long-term goals.

Interdisciplinary Collaboration : Lead meetings with multidisciplinary teams, caregivers, and families to coordinate resident care.

Community Engagement : Work with specialists to optimize resident outcomes.

Leadership & Education : Guide nursing staff, communicate effectively at all levels, and conduct in-service education on MDS, RAPS, and Triggers.

Schedule Adherence : Maintain compliance with the MDS / Care Planning schedule for non-Medicare residents.

Qualifications :

Florida RN License : Valid and in good standing.

Skilled Nursing Experience : Minimum of 2 years in a Skilled Nursing Facility.

Expertise : Strong knowledge of MDS, Medicare, and the care-planning process.
