Now Hiring : Full Time Physical Therapist (PT)

Service Area : Melbourne to Palm Bay / Micco to include adjacent beachside areas

Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we’re looking for Full Time Physical Therapist (PT) to join our award-winning team. .

Key Responsibilities

• Perform initial and ongoing patient assessments, including OASIS evaluation and documentation.
• Develop and implement tailored treatment plans in consultation with physicians and clinicians, adjusting as needed.
• Collaborate with the interdisciplinary care team to optimize patient outcomes.
• Provide education and support to patients and families / caregivers on therapy programs.
• Maintain accurate clinical records and report any changes in the patient’s condition to the physician.
• Supervise Physical Therapy Assistants in accordance with agency policies and state regulations.
• Participate in in-services, case conferences, and meetings to promote coordinated care with healthcare team members.
• Identify patient and family / caregiver needs for additional services and make appropriate referrals.
• Confirm daily visit schedules to ensure seamless coordination with other staff.
• Communicate patient progress and changes with physicians and care teams.
• Document all visits and progress notes on the same day of service.
