Job Title : Nurse Practitioner / Physician assistant - Home Health Risk Assessments
Overview : Our client is a leading healthcare organization dedicated to providing exceptional care to patients in the comfort of their homes. We are currently seeking skilled Nurse Practitioners or Physician assistants to join our team.
Job Details
- Part-time : 1-2 days a week or 25-30 assessments per month, with scheduling based on needs and NP's availability (needs may fluctuate throughout the year).
- M-F, 8A-8P availability.
- Projected to be a 3–4-month contract (may extend based on member’s needs).
- Compensation : Best rates per assessment, plus mileage reimbursement after 30 miles driven for the day.
Job Description : As an in-home health assessment provider, you will be responsible for conducting Health Risk Assessments (HRAs) through house calls within an assigned area. In this role, you will :
Travel to patients' homes, nursing homes, or other health facilities to perform comprehensive history and physical assessmentsDocument acute, chronic, and long-term healthcare issues accurately and efficiently using our EMR systemEnsure all appropriate consent forms are signed and datedInteract frequently with patients, coders, schedulers, and supervising physiciansEducate patients and their families on chronic medical conditions, preventative care, and medication adherenceComplete referrals to case management, nursing, pharmacy, and other services as neededParticipate in clinical meetings and compliance trainings (primarily web-based, with occasional on-site training)Review patient records for past and present health issuesCollaborate with the supervising physician and other members of the healthcare teamInterpret laboratory data and diagnostic testing when available to assist in diagnosing medical conditionsPerform ADL and Fall Risk evaluations, Depression and Nutritional Health Screenings, BMI evaluations, and medication reconciliationsPerform additional duties as assigned by the supervising physician, such as diabetic retinal exams, bone density scans, etc.Qualifications
Family Nurse Practitioner certification required (Adult NP may be considered if no pediatric patients will be seen)Board certification by the ANCC or AANP or Physician Assistant equivalent.Active / unrestricted NP or PA license within your state of practice (new graduates will be considered)Credentialed by Medicare, Medicaid, and other private insurance companiesProficiency with various computer-based applications (MS Office, logging into programs, navigating different software, typing) and utilizing websites to perform required dutiesStrong background or interest in primary care and preventative medicine (preferred)Access to a personal car with adequate automobile insuranceHighly motivated, self-directed professional with strong organizational skills and the ability to work independentlyHome internet connectionExcellent clinical assessment and analytical skillsDEA certification may be required depending on state of practice.Please submit your resume for immediate consideration.
