JOB SUMMARY :
The Registered Nurse On Call (RNOC) plans, organizes and directs hospice care and is experienced in nursing, with the emphasis of serving patients while on call. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Territory : Salt Lake and Utah County
Schedule : 7 on / 7 off
MAJOR JOB FUNCTIONS :

- Follows the initial, comprehensive, and ongoing comprehensive assessment of patient and family established by the Registered Nurse Case Manager (RNCM) to determine hospice needs. Provides a physical assessment and history of current and previous illness(es).
- Provides professional nursing care by utilizing all elements of nursing process.
- Assesses and evaluates patient’s status by :
- Following plan of care and notifying Interdisciplinary Group (IDG) of needed updates because of after hour on- call interventions.
- Evaluates patient and family / caregiver needs when called afterhours.
- Uses health assessment data to determine needs on-call.
- Initiates appropriate preventative nursing procedures and on-call medications and treatments as prescribed by the physician in the physician’s plan of care.
- Counsels and provides education to the patient and family / caregiver during periods of crisis to facilitate decision making and allow the patient to remain in the care setting of their choice.
- Provides health care instructions to the patient as appropriate per assessment and plan.
- Assists the patient with activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care.
- Completes, maintains, and submits accurate and relevant clinical notes regarding patient’s condition and care provided. Records pain, symptom management changes, interventions, and outcomes as appropriate.
- Communicates with the physician regarding the patient’s needs and reports changes in the patient condition; obtains, receives, and documents physician’s orders as required.
- Educates the patient and family / caregiver self-care techniques and other task as they relate to the patient’s plan of care.
- Provides medication, diet and other instructions as ordered by the physicians.
- Recognizes and utilizes opportunities for health counseling with patients and families / caregivers.
- Works in concert with the interdisciplinary group (IDG).
- Provides and maintains a safe environment for the patient.
- Assists the patient, family or caregiver and other team members in providing continuity of care.
- Works in cooperation with the family / caregiver and hospice IDG to meet the emotional needs of the patient and family or caregiver.
- Ensures that arrangements for equipment and other necessary items and services are available during on-call visits.
- Assumes responsibility for personal growth and development maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
- Fulfills the obligation of requested phone triage registered nurse.
- Performs other duties as assigned.
REQUIRED EDUCATION / EXPERIENCE :
Registered Nurse of an accredited school of nursing with a current license to practice professional nursing in the state in which we do business.One (1) year of acute care experience in long term care, hospice, or home health setting.Must have reliable transportation, proof of current automobile insurance and a valid driver’s license.SKILLS / ABILITIES / KNOWLEDGE :
Ability to market and deal tactfully with customers and the community.Demonstrates the ability to clearly document according to standards of practice and to accurately and efficiently audit the documentation of others utilizing resources and tools provided.Demonstrate strong communication skills, relationship building, and consistently produce work that is detail-oriented and accurate.Possesses a professional and resourceful style; having the ability to work independently and as a team player, to take initiative, and manage multiple tasks at the same time.Proficient in the Microsoft Office suite of products and related computer skills.Complies with acceptable professional standards and practice.Demonstrates ability to supervise and direct others.Excellent observation, written, verbal and communication skills with good nursing judgment.Understands hospice philosophy as it relates to end of life.PHYSICAL DEMANDS :
Ability to stoop, bend, kneel, crouch, or climb stairs.Stand, walk, and / or sit for extended periods of time.Able to move about on rough surfaces.Talking, hearing, vision acuity, and speaking within normal ranges is essential for normal conversations, to receive ordinary information and to prepare or inspect documents necessary to perform nursing duties.Lifting of up to 50 lbs. occasionally may be required.Endure long periods of driving.Good manual dexterity for the use of common office equipment such as laptop, calculator, copier, printer, and fax machine.Because those who care deserve more.
