Position Summary
Prestige Care – Fairfield is seeking a Per Diem Occupational Therapist (OT) to join our dynamic in-house subacute rehabilitation team. The OT will assess, plan, and deliver therapy services to help patients regain and maintain their ability to perform daily activities following illness, injury, or surgery. This role plays an essential part in promoting independence and successful recovery within our skilled nursing environment.
Key Responsibilities
Conduct comprehensive evaluations of residents’ functional abilities and needs related to activities of daily living (ADLs), fine motor skills, and upper extremity function.
Develop and implement individualized treatment plans in accordance with physician orders and patient goals.
Provide hands-on occupational therapy interventions to improve strength, coordination, range of motion, and functional independence.
Educate patients, families, and caregivers on home programs, adaptive techniques, and use of assistive equipment.
Collaborate closely with an interdisciplinary team—including PTs, SLPs, nurses, and social workers—to support holistic patient care.
Accurately document evaluations, daily treatment notes, progress reports, and discharge summaries in the electronic health record.
Participate in team meetings, case conferences, and continuing education to maintain clinical excellence.
Ensure compliance with state and federal regulations, professional standards, and facility policies.
Qualifications
Required : Bachelor’s, Master’s, or Doctorate degree in Occupational Therapy from an accredited program.
Required : Current Iowa Occupational Therapist license (or eligibility).
Preferred : Experience in subacute, skilled nursing, or long-term care settings.
Strong interpersonal, documentation, and time management skills.
