Job description

Your Role as an RN :

  • Conduct patient visits as assigned, performing assessments, eligibility determinations, admissions processing, care planning, implementation, and evaluations.
  • Gather essential data to assess the physical, psychological, social, and spiritual needs of patients and their families, coordinating with the interdisciplinary team.
  • Communicate with physicians and other care team members to ensure optimal care delivery.
  • Regularly evaluate and update care plans based on patient needs.
  • Execute therapeutic, preventable, and rehabilitative nursing procedures as prescribed.
  • Provide clinical pronouncements of death and emotional support to families, when applicable to licensure.
  • Maintain accurate and timely documentation of patient records and care plans.

    • Qualifications :

  • Must be a Registered Nurse, having graduated from an accredited nursing program and hold a valid state license.
  • At least one year of clinical experience, ideally in intensive care, rehabilitation, or medical-surgical nursing.
  • Home health experience is a plus!
  • Valid CPR certification is required.
  • Must have reliable transportation, a current driver's license, and valid automobile insurance.
