Oregon Licensed Practical Nurse

A company is looking for a Licensed Practical Nurse (LPN) with Oregon Licensure for a remote day shift position.

Key Responsibilities

Manage patient census to close gaps in clinical and non-clinical care

Identify and coordinate community resources for patient care

Provide education on managing chronic conditions and perform medication management

Required Qualifications

Active Oregon License (LPN)

At least 3 years of direct patient-facing work experience

Technical aptitude with Microsoft Office Suite

Experience in chronic care management

Ability to work in a remote, HIPAA-compliant environment
