Licensed Practical Nurse (LPN) - Full Time Position\n\nSchedule Details\n \n
Full Time - 3x12 hour shifts (36 hours total)\nShift hours : 4 : 30am - 4 : 30pm\nWeekend rotations required\n\n\nRequired Qualifications\n \n
NY LPN Licensure\nBLS certification (AHA or RQI specifically)\n1 year experience as LPN\nMust have Dialysis experience\n\n\nPosition Overview\n \n
Demonstrate knowledge and skills necessary to provide care appropriate to the age of patients served\nKnowledge of principles of growth and development across the life span\nAbility to assess data reflective of patient's status and interpret appropriate information\nIdentify each patient's requirements relative to age-specific needs\nProvide care needed by adult and geriatric patient groups\nProvide direct and indirect skilled nursing care for assigned residents\nFollow defined policies and procedures under supervision of a Registered Nurse\nMaintain collaborative relationship with other nursing staff, physicians, and ancillary staff\nMaintain appropriate level of knowledge and clinical expertise through continuing education and in-service programs\nAbility to provide care to geriatric residents\n\n\nJob Duties\n \n
Collect data relevant to resident's physical and psychosocial condition on an ongoing basis and communicate findings to RN and other members of the Interdisciplinary Team\nContribute to the creation of a developmentally age appropriate and culturally relevant plan of resident care reflecting nursing diagnosis and interventions to achieve desired resident outcomes involving the resident / family and healthcare team in this process while promoting a healthy, safe environment\nRecommend revisions to the plan of care, after evaluation of resident outcomes and indicate the need for referrals to ensure all aspects of the resident's care have been addressed\nCollaborate with the resident / family and members of the Interdisciplinary Team within the scope of nursing practice assuring customer satisfaction\nMaintain current knowledge and competency in Nursing Practice\nParticipate in the mentoring / orientation of staff\nConsider factors related to safety, cost, and impact in planning in delivery of care\nUnder the direction of the RN, provide leadership in the professional practice setting\nDemonstrate knowledge and competency\n\n\nAdditional Information\n \n
