Position Summary
This position is dedicated to developing, implementing, and evaluating innovative educational plans and programs tailored to meet the diverse needs of patients. We adhere to established best practices, standards, and regulations to ensure optimal patient care.
Position Duties
- Collaborate with colleagues within the Endocrine Department to align with strategic goals and support institutional growth initiatives.
- Work alongside various disciplines and departments as an advocate for change, actively participating in the evaluation of health care outcomes.
- Conduct thorough assessments of patient conditions—taking into account age, physical health, psychological status, and educational needs—to establish compliance with treatment plans. Regularly perform follow-up assessments to identify variations from the diagnosis.
- Continuously monitor patient responses to care plans, educational initiatives, and discharge operations. Document progress in detailed charts and effectively communicate observations and assessments with the healthcare team and families to ensure comprehensive understanding of patient health status and ongoing needs.
- Contribute to team efforts by completing required activities according to established standards to maintain compliance with medication and documentation protocols.
- Engage in continuous improvement efforts that embody the core philosophy and values of our organization, promoting professional development among staff.
- Design, implement, and assess educational opportunities utilizing evidence-based practices and diverse teaching strategies. Actively seek opportunities for personal and professional growth.
- Oversee patient care activities carried out by assigned team members, delegating tasks appropriate to their skills and competencies. Provide feedback to ensure safety, infection control, and high-performance standards, while maintaining confidentiality.
- Collaborate with the patient care team and family members to formulate and execute care plans, ensuring coordinated communication regarding the patient's health status. Participate in patient care rounds and conferences as required.
- Manage patient cases effectively, ensuring comprehensive support throughout their healthcare journey.
