Referral Specialist

Job Summary : Under the direction of the Population Health Manager, the Referral Specialist works as part of a patient care team to provide high quality, efficient and service oriented patient care while demonstrating the health center's core values.

Responsibilities

• Patient Relations – Work cooperatively with patients, medical providers, health center staff and community stakeholders in all aspects of patient care and care coordination.
• Patient Referrals – Obtain and process referrals from providers, prepare and complete referrals accurately, track referrals in the electronic medical record system, monitor and report on authorization status, and manage re-authorization to avoid lapses.
• Provider & Team Support – Participate in daily morning care team huddles, screen and answer referral calls, serve as the main point of contact for providers and clinic staff regarding referrals and scheduling, and act as a liaison between participants, providers and community resources.
• Additional Responsibilities – Perform other duties as assigned by the supervisor.
