Per Diem Physical Therapist (PT) - Home Health - Chula Vista Area

JTC Home Health Inc. is expanding our exceptional therapy team and seeking a motivated, compassionate Per Diem Physical Therapist to provide top-quality care to our patients in the Chula Vista area and surrounding communities in San Diego County.

🏡 Setting: Home Health 📍 Area: Chula Vista, CA (and nearby South Bay/San Diego County) 💵 Pay: Competitive Per Diem Rate (based on experience and visit type)

Key Responsibilities

Perform comprehensive physical therapy evaluations to assess patient condition, functional abilities, and home environment.

Develop and implement individualized Plans of Care (POCs) in collaboration with the physician and interdisciplinary team (RNs, OTs, SLPs).

Provide skilled physical therapy treatments, including therapeutic exercise, gait training, and instruction on the use of assistive devices.

Educate patients and caregivers on safe and effective home exercise programs and disease management.

Timely and accurate completion of all required documentation using an Electronic Medical Record (EMR) system.

Adhere to all state and federal regulations for home health care.

Qualifications

Current and unrestricted Physical Therapist (PT) license in the State of California.

Doctor of Physical Therapy (DPT) degree preferred.

Minimum of 1 year of professional experience as a Physical Therapist (home health experience is a plus!).

Current CPR certification.

Valid driver's license, reliable transportation, and auto insurance.

Excellent clinical, communication, and time-management skills.
