Home Health Licensed Practical Nurse

This role offers the opportunity to provide meaningful, patient-centered care in a home setting while collaborating with a supportive multidisciplinary team. The ideal candidate is dependable, organized, and committed to delivering high-quality nursing care.

Responsibilities include assisting in the management and delivery of individualized care plans, providing direct patient care, documenting clinical information accurately, and ensuring charting deadlines are met. The LPN will also support patients across various Pennsylvania locations and utilize technology tools to maintain communication and documentation.

Schedule : Full-time hours, Monday through Friday. Specific schedule and shift expectations will be provided during onboarding.

Travel : Occasional local travel required within assigned service areas.

Required qualifications :

• Active Licensed Practical Nurse (LPN) license in the State of Pennsylvania
• Associate degree or higher in Nursing
• Minimum of two years of LPN experience (Home Health experience preferred)
• Experience using Electronic Medical Record (EMR) systems such as TruChart
• Current AHA BLS and ACLS certifications
• Valid driver’s license, reliable transportation, and auto insurance
