Homebase Medical is growing in the  Inland Empire area.  We are currently searching for  an experienced Nurse Practitioner or Physician Assistant  to engage in Medicare Wellness Visits for full-time hours at our contracted partner clinics and the occasional home visit.

  • This is a full-time position of 5 days per week, 8-hour days (8am - 5pm / 40 hours per week)
  • You will travel to clinics in Lake Elsinore, Hemet, Moreno Valley and Perris, CA.

Responsibilities include :

  • Perform Wellness Visits in partner primary care clinics and facilities in the Inland Empire community.
  • Drive to designated facility locations in your assigned geographic area.
  • Review patient medical and family history.
  • Review of current providers and prescriptions.
  • Perform height, weight, blood pressure, and other routine measurements.
  • Deliver personalized health advice.
  • Review risk factors and potential treatment options.
  • Exhibit stellar bedside manner with the ability to communicate complex medical information with simplicity and kindness.
  • Provide health education to patients. Work with patients in the prevention of illness, promotion of health, and management of patient's adjustment to chronic health problems.
  • Provide referrals to appropriate medical resources.
  • Utilize our EMR system to document patient visit / interaction accurately and timely.
  • Knowledge of HCC coding.
  • Other duties as assigned.

    Qualifications and experience :

  • Graduate of an accredited Physician Assistant program or Nurse Practitioner program required.
  • Current, unrestricted NP or PA license to practice in California, including DEA license.
  • Minimum of 2 years of experience in primary care, hospice, or urgent care / outpatient setting.
  • Minimum of 1 year of previous experience utilizing an EMR system.
  • Spanish speaking preferred.
  • Good working knowledge of HCC coding.
  • Strong computer skills including MS Office and Outlook is strongly desired.
  • Great verbal and written communication skills
  • Current drivers’ license, auto insurance and reliable transportation

  • Base salary range :   $140,000 - $150,000 annually
