The Care Manager is a professional with specialized skills in the psychological, emotional and social aspects of geriatric client care, with a working knowledge of community and social resources. They should have a sensitive understanding of the special needs of clients with chronic, treatable illnesses and the elder population.
Geriatric Care Manager Primary Functions :
- Manages a caseload of 10. Clients who have chronic illnesses and need assistance in obtaining community resources and addressing psychosocial issues.
- Completes comprehensive assessments on a timely basis as per the admissions and readmissions process.
- Assesses the social and emotional factors related to the client chronic illnesses.
- Coordinates plan and provides counseling and support to the client and family based on an assessment.
- Completes treatment plan with clients and families to assist with interaction of client.
- Assesses financial situation of clients as appropriate, interpreting the medical situation and referring clients / families to community agencies as required.
- Identifies environmental safety hazards and risk factors (emotional suicidal individuals) and educates the client / family accordingly.
- Completes all required documentation in a timely and accurate manner.
- Enters all relevant information and notes into MyJunna, Electronic Medical
- Records / Billing System in a timely manner.
- Assist with determining outcome based criteria for GCM role and evaluate overall program effectiveness.
- Provides community outreach and works with Director to build case management census.
- Attends all mandatory meetings
Geriatric Care Manager Qualifications :
Master’s Degree in Social Work OR Nursing preferred or similarMust have at least 2 years of clinical geriatric care management or related experience.Must be a Certified Geriatric Care Manager or meet the qualifications to sit and pass certification exam within reasonable and agreed upon timeframe.Must demonstrate the following qualities :A presentation of self that is friendly, caring and cooperative.An approach to client needs and problems that is organized and thoroughA flexible and creative approach to clients and care plan developmentThe ability to assist clients and families in making decisions without imposing their own opinions or being judgmentalAn understanding of their value as a GCM and feeling comfortable in charging fees for their time and expertiseThe ability to delegate tasks that are appropriate to paraprofessionals or non-professionals.Demonstrate an excellence in both written and oral communications.Company Description
