Job descriptionConduct comprehensive annual home health risk assessments (45 minutes average). Evaluate patients' medical history, conditions, and medications. Educate patients on health conditions and medications. Identify gaps in care and provide recommendations. No creation of treatment plans or prescribing medications required. Flexible! Create your own schedule with shifts available Monday-Sunday. Minimum weekly or bi-weekly hourly commitment. NY Nurse Practitioner License. Current CPR certification (BLS). Preferred : 6 months of current NP experience. Preferred : Board Certification through AANP or ANCC. MSN degree from an accredited school of nursing. Competitive hourly rate : $80-$90 per evaluation + mileage stipend + administrative time. Flexible schedule allows you to control your workload. W-2 based position with benefits.
