Community Integration Counselor (HCBS Overnight)

Community Options, Inc.  is a national non-profit agency providing services to individuals with disabilities in 12 states.

We are now hiring a  Part-Time Overnight Community Integration Counselor  in the  Greater Phoenix, AZ  area. The Community Integration Counselor (CIC) is responsible for assisting individuals with learning physical, intellectual, emotional, and social life skills. The Community Integration Counselor provides individualized services designed to assist participants in managing and overcoming the difficulties confronted when living in the community.

Location : Ahwatukee Foothills

Shift : Tuesday & Wednesday 4 : 00PM-8 : 00AM

Pay : $17.00 per hour

Responsibilities

• Provide daily personal care and assistance with activities of daily living - Evening / Overnight Shift (This is a Non-Sleeping shift)
• Maintain a clean, well-ordered environment and follow established household routines
• Monitor for seizure activity and respond quickly and appropriately
• Document care activities and observations clearly and accurately
• Support health and wellness through medication reminders and communication with family and medical professionals
• Administer medications and ensure proper completion of necessary documentation
• Coordinate all leisure and volunteer opportunities
• Monitor the health and medical needs of individuals and immediately report any concerns
• Drive individuals to medical appointments and activities in the community
• Ensure program documentation and billable records are completed accurately and timely
• Offer companionship and encourage independence where appropriate
• Additional tasks and responsibilities may be assigned
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation
