NOW HIRING: Social Worker
Responsibilities:
Conduct comprehensive psychosocial assessments of residents to identify their emotional, social, and environmental needs.
- Develop individualized care plans in collaboration with interdisciplinary teams to address residents' social and emotional concerns.
- Provide counseling and emotional support to residents and their families, especially during times of transition or crisis.
- Coordinate discharge planning and arrange for appropriate community resources and services for residents transitioning to other care settings.
- Advocate for residents' rights and ensure their voices are heard in care planning meetings.
- Collaborate with healthcare professionals, including nurses, physicians, and therapists, to ensure holistic care for residents.
- Document assessments, interventions, and progress notes accurately in resident records.
Qualifications:
- Current licensure or eligibility for licensure as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in New Jersey.
- Previous experience in a nursing home or long-term care setting preferred.
