A unique opportunity is available for a Clinical Pharmacist to support a hybrid clinical and staffing role in Lewistown, PA. This position focuses approximately 60% on clinical responsibilities and 40% on staffing duties, offering a balanced work experience with potential for program growth.

Responsibilities :

• Provide clinical pharmacy services with a focus on Population Health and Ambulatory Care.
• Manage medication regimens for patients, emphasizing care for target disease states such as diabetes.
• Support staffing needs, including dispensing and medication management activities.
• Collaborate with healthcare teams to optimize patient outes.
• Contribute to the expansion of clinical pharmacy services within the health system.
