Requirements  Saint Alphonsus Medical Center Nampa is looking for an Interventional Cardiologist to fill their need!Schedule : Call coverage ONLY No ClinicCurrent Coverage Needed : 24 hour call12 / 2 7a - 12 / 8 7a12 / 23 7a - 1 / 5 7a1 / 22 7a - 1 / 28 7a2 / 18 7a - 2 / 24 7a3 / 19 7a - 3 / 25 7a 4 / 16 7a - 4 / 21 7aDetails : Inpatient onlyPatient volume varies - 3 consults per day 2 procedures per dayTrauma level II18 ICU beds 18 tele beds and 10 CMU bedsAPP support - APP's- M-F 1-2 APP's are staffed during the day weekends- 1 APP for 5 hours / dayInterventional cardiology procedures requiredTEE's would be highly preferred. Balloon pumps coronary stents IVUs and IFR are important.STEMI volumes vary greatly. In 2020 they averaged 3 STEMI activations per weekTravel Requirements : Flights - 750 / round trip cap to include all taxes and feesLodging - 179 / night cap for hotels to include all taxes and fees. Preferred Lodging : Candlewood Suites Boise – Towne Square an IHG Hotel.Rental Car - To be reimbursed by facility not to be used with personal car / mileage reimbursement. 50 / day cap including all taxes and fees regardless of car class. SUV / upgrades over cap will need to be preapproved will only be approved if needed for inclement weather.Mileage - IRS Standard Rate to be reimbursed by facility only to be used with personal vehicle not to be used with rental car.Bill Rates Needed for Submission : Malpractice must be included in the ratesOrientation24 Hour On Call 4800 rate cap with 10 hours gratisHoliday 24 Hour On Call Weekend Call Holiday Weekend Call Call Back Holiday Call BackConversion fee 25k rate capPlease make sure you have the following items readily available for your provider at the time of acceptance in order to expedite the credentialing processMD diplomaInternship / residency / fellowship certsAll medical licensesDEABoard CertificationLife support cards- BLSCurrent and previous certificates of insuranceDriver’s licenseProfessional color photo / headshotCMEs for the past 2 yearsImmunization records including TB Flu HepB Tdap Varicella and COVIDReferencesCase logs
