The Hospice Nurse Case Manager admits patients to service.

They assess the patient's and family's physical needs.

They evaluate psychosocial, environmental, safety, and developmental needs.

They develop an individualized plan of care based on their assessment.

They document observations and interventions related to the patient's environment.

This role coordinates care with a multidisciplinary team.

They participate in the hospice program's quality assessment performance improvement program.

They conduct follow-up visits as needed.

They are responsible for addressing the needs of patients in the designated territory.

Holiday coverage may be required.
