The Director of Medical Services will directly supervise and mentor primary care providers (PCPs) at their assigned center. You will play a crucial role in maximizing the execution of our core model, focusing on enhancing clinical quality, efficiency, patient outcomes, and clinician satisfaction. A portion of your time will be dedicated to providing direct patient care as a PCP, while the rest will focus on leadership tasks, including managing and engaging with the PCP team to foster a vibrant clinical-operations culture.

Key Responsibilities :

• Deliver exceptional care to older adults with acute and chronic illnesses.
• Take full accountability for patient care and outcomes, ensuring effective collaboration with specialists when necessary.
• Engage actively with hospitalists when your patients require hospitalization.
• Conduct thorough assessments, diagnoses, treatments, management, and education for patients with complex health needs.
• Lead a dedicated care team consisting of care promoters, facilitators, and coordinators to support patients who can visit the office.
• Collaborate with transitional care teams for patients unable to visit the office due to hospitalization, skilled nursing facility stay, or other circumstances.
• Lead weekly clinical meetings like Super Huddles (SH) and Transforming Care Meetings (TCM), and support center operations as needed.
• Actively recruit patients and interview potential PCP candidates for the company.
• Mentor and coach PCPs to ensure outstanding clinical outcomes.
• Partner with Center Operations Directors and General Managers to enhance center financial performance and grow patient membership.
