Responsibilities

The Home Health Aide provides personal care and related services, focusing on activities of daily living, in the home for the sick or disabled. As a Home Health Aide you will :

• Provide assistance with activities of daily living (i.e., personal hygiene) as part of the agreed-upon plan of care
• Understand and apply basic infection control principles and procedures
• Keep a record of HHA Plan of Care for each patient assigned and provides updates to assigned case manager and patient's family
• Attend required in-services and meetings
• Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
