Position Summary

The Community Health Navigator will help recruit patients for Apicha CHC's Jackson Heights health center, particularly outreaching to Bangladeshi and / or Filipino communities in Western Queens. This role involves working closely with staff from different departments to make sure patients are smoothly connected to care. This role will conduct outreach to Asian Pacific Islanders communities in Western Queens and promote Apicha CHC's services. The role also includes assisting with administrative tasks.

The major duties of the position include, but are not limited to :

• Outreach - Recruit patients from priority populations
• Education - Educate our priority communities on various health and mental health topics.
• Client Service - Help individuals and families access health services in a culturally and linguistically appropriate way.
• Compliance - Follow policies and procedures for data entry, reporting, and meeting deadlines.
