Home Health Physical Therapist

Full Time | Flexible Schedule | Primarily Day Shift

Home Health Physical Therapist Qualifications :

Doctor of Physical Therapy (DPT) degree from an accredited program.
Active Physical Therapy license in the state or eligibility to obtain one.
Home health experience preferred but not required.
CPR certification required.
Valid driver's license and reliable transportation.

Join our dedicated home health care team as a Home Health Physical Therapist to deliver quality care directly to patients in their homes. This role emphasizes personalized in-home rehabilitation to improve patients' mobility, function, and quality of life. Ideal for professionals who enjoy a flexible schedule and building meaningful relationships with patients in a comfortable home setting.

Home Health Physical Therapist Key Responsibilities :

Conduct thorough assessments to understand each patient's physical condition, needs, and goals for recovery.

Design and implement personalized treatment plans that include therapeutic exercises, mobility training, and other physical interventions to restore function and relieve pain.

Educate patients and caregivers on techniques for exercises, mobility aids, and activities of daily living to promote safety and independence.

Collaborate with a multidisciplinary team , including doctors, nurses, and social workers, to develop comprehensive care plans tailored to each patient's needs.

Travel to patients' homes within the service area, ensuring timely and reliable care delivery.
