Ohio Licensed Ambulatory Care RN

A company is looking for a Registered Nurse Ambulatory Care Manager (RN) - Population Health.

Key Responsibilities

Provide clinical care management services to eligible patients, coordinating care for desired health outcomes

Develop and implement care plans, perform medication reviews, and collaborate with healthcare providers

Manage patient outreach, referrals to resources, and document all communications in electronic medical records

Required Qualifications

Associate's Degree in Nursing (required)

Bachelor's Degree in Nursing (preferred)

RN compact license or Multi-State License (required)

2-3 years of acute care, home health, or case management experience

Case Management certification (preferred)
