Job Summary :
The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Strong communication and collaboration with patients, providers, and other members of the team is critical.

Responsibilities :
- Provide skilled nursing care to patients with various medical conditions.
- Conduct patient assessments and develop individualized treatment plans.
- Administer medications and treatments as prescribed by physicians.
- Collaborate with the interdisciplinary team to ensure comprehensive patient care.
- Document patient care accurately and timely.
- Participate in case management and discharge planning.
- Provide education and support to patients and their families.
