Duration - 3 months | CTH

Shift - M-F 8a-6pm Central time shifts will be during those hours.

Option to work on weekends (occasionally)

Responsibilities

Remote data verification and prescriber / patient calls for home infusion, IVIG, and hemophilia orders

The team is currently a relatively small one used to supporting each other and working together well.

Receiving / clarifying prescriptions from provider's office with all necessary information for home infusion including but not limited to : meds prior to infusion, flushes, calculate infusion time, nursing and all necessary supplies / pump.

Counseling and providing care plans to patients as well as support for providers and home infusion nurses

Expectations : to adapt to changing workflow, prioritizing tasks, can take initiative and work independently while maintain cohesion with the team

Providing individualized clinical care to patients with rare disease states

Inbound / outbound calls

Education / Certification Required :

Bachelor's degree in pharmacy and 3+ years of mail order, retail or hospital pharmacy experience OR PharmD

Preferred : IGNS certification

Licensure Required :

Active Pharmacist License in TX, MO, or FL.

Nice to haves : IGNS certification

Infusion therapy experience
