MedAdventures is looking for a dedicated Home Health Physical Therapist to join our Mifflin, PA team. In this role, you’ll provide personalized, one-on-one care in patients’ homes, helping them regain mobility, restore independence, and improve their overall quality of life. Recent home health experience with OASIS evaluations is required.

    • Your Responsibilities :

  • Conduct comprehensive home health evaluations using OASIS
  • Develop and implement individualized treatment plans
  • Deliver skilled PT interventions directly in patients’ homes
  • Document patient progress thoroughly and promptly
  • Collaborate with nursing, OT, and other team members to achieve optimal outcomes
  • Educate patients and caregivers on exercises, safety, and recovery strategies

    • Qualifications :

  • Active Pennsylvania PT license (or eligibility to obtain)
  • Minimum of one year of recent home health experience
  • Proficiency in OASIS documentation
  • Strong clinical reasoning, organizational, and communication skills
  • Commitment to providing compassionate, patient-centered care
