Position Summary
The Counselor is responsible for gathering data, documenting problems and establishing course of treatment. Provides direct addiction treatment services to adult patients and family members.
Essential Duties and Responsibilities
- Provide individual and group counseling to patients assigned to caseload.
- Direct training groups and seminars. Provide therapeutic leadership in group counseling sessions.
- Function as a liaison with other community-based agencies and law enforcement agencies to insure continuity of treatment (referrals) and compliance to legal obligations.
- Complete individual chemical dependency assessment of new patients, including psychosocial assessment and required forms for signature. Ensure accuracy and completion of the assessment process.
- Develop an Individual Treatment Plan (ITP) for new patients within the established time frame, with patient input and signature.
- Update patient charts within established timeframes; ensure treatment plans reflect current problems / goals and that goals are expressed in the patient\'s own words.
- Work with medical staff to secure urine samples for monthly urinalysis and document results in the patient's chart and program software.
- Participate in Diversion Control through execution of random bottle recalls.
- Provide necessary forms to the Medical Director for take-out medications, dose changes, phase changes, MSW schedule, and medication changes.
- Ensure patient confidentiality by properly completing release of information for outside referrals and contacts (e.g., medical coordination, information from other facilities).
- Maintain assigned patient files in compliance with DHHS regulations and program policies; keep files secured at all times.
- Complete discharge paperwork within seven days of discharge and submit the file to the Program Director.
- Contribute to ongoing assessment and treatment planning through daily reports, case reviews, and treatment teams.
- Provide patient education relating to chemical dependency and the recovery process.
- Provide supportive counseling or crisis intervention as circumstances require, in conjunction with medical and nursing services.
- Provide education and public information to the general public and professional community regarding opioid treatment programs.
- Participate in ongoing professional development and pursue credentials; engage in CQI monitoring and reporting as assigned.
- Participate in the Continuing Quality Improvement program and report on assigned CQI monitors.
Job Requirements / Qualifications
Minimum Qualifications : Four-year degree in counseling or related field, or equivalent experience. Must possess an active license to practice in the state of NM. Acceptable licenses include LSAA, LADAC, LMHC, LPCC, LMFT, LMSW or LISW. Must have a valid driver's license or state-issued ID.
