About The Position

ID : 73557051 Shift : Working 36 -40 / hours per week, working days with rotating weekends Description : This Traveler Assignment will be for our 78-bed licensed Facility to work alongside our RN Case Managers. Position may include working in the inpatient or emergency department. Assesses patients and / or patient's family, caregivers, and / or legal representatives and arranges for needed interventions. Plans care-focused interventions to help patients cope with physical, social, emotional, economic, and environmental concerns and provides treatment planning for support. Responsibilities : Provides psychosocial support to patients through direct counseling and collaborative visits, with cultural humility and sensitivity. Performs and documents care psychosocial assessments. Collaborates and serves as a resource for social worker team members working with patients in the hospital. Participates in resourcing of outpatient services relevant to patients. Coordinates educational and outreach services. Within scope of job, requires critical thinking skills, decisive judgement and the ability to work with minimal supervision. Must be able to work in a fast-paced environment and take appropriate action. Requirements : Master's degree in Social Work. State licensure as a Licensed Clinical Social Worker (LCSW). Minimum of 3 years of clinical experience in the hospital setting. Discharge planning experience Required.
