Home Health RN Responsibilities

• Provide skilled nursing care in the patients home setting includingassessments treatments wound care and medication management.

• Develop and implement individualizedhome health care plans adjusting as patients conditions change.

• Educate patients and families on health conditions treatments and at-home care.

• Collaborate with theDirector of Nursing physicians and interdisciplinary teamsto coordinate quality care.

Qualifications for Home Health RN Jobs

• Graduate of an accredited nursing program (BSN preferred).

• Current California Registered Nurse (RN) license in good standing.

• At least 1 year of professional nursing experience (home health nursing experience strongly preferred).

• Public Health Nurse (PHN) certification a plus.

• Strong clinical judgment communication and organizational skills.

• Valid drivers license reliable transportation and ability to travel to patient homes in North Orange County areas.
