RN Service Coordinator Job Details :
Schedule : Monday - Friday, 8 : 00am - 5 : 00pm
Location : Field visits. (45 South area, Webster, Friendswood, League City, Galveston, Clear Lake, Pearland, Midtown, Downtown)
Temporary to hire
Starting Pay Rate : $40.38 / hour + (based on experience)

RN Service Coordinator Responsibilities :
- Perform telephonic and face-to-face clinical assessments to identify and manage members' physical, behavioral, and social service needs.
- Obtain member history to develop and implement individualized care plans in collaboration with members, caregivers, and providers.
- Identify high-risk members and coordinate care with the healthcare team to reduce complications and improve outcomes.
- Supervise and direct LPN / LVN, LSW, LSCW, LMSW, and non-RN clinicians in accordance with state laws and organizational policies.
- Interface with Medical Directors, Physician Advisors, and interdisciplinary teams to review and adjust care management treatment plans.
- Facilitate authorizations and referrals for services, ensuring compliance with benefit structures and regulatory standards.
- Provide mentorship and training to new staff and contribute to process improvement initiatives.
- Participate in quality assurance activities, audits, and internal reporting processes.
- Travel locally to member homes and community settings as needed to perform assessments.
Education Requirements :
High school diploma or equivalent, required.Active, unrestricted RN license in the State of Texas (RUG certified) required; NP license accepted.Bachelor of Science in Nursing (BSN) and Certified Case Manager (CCM) preferred.Job Requirements :
3-5 years of nursing experience in case management, service coordination, or care management involving chronic illness, disabilities, or long-term care.1+ years of experience working with populations receiving waiver services preferred.Proficiency in Microsoft Office and clinical documentation platforms.Strong organizational, communication, and critical thinking skills.Reliable transportation with a valid driver's license and good driving record.Know someone who may be interested?
