Description : Unit Notes :
Start dates are not flexible.
A 100-mile radius rule applies.
No 48-hour guarantees or blocked scheduling requests.
No more than five days of RTO is allowed.
Nurses must work two of three holidays : Thanksgiving, Christmas, and New Year's.
Nurse-patient ratios are typically 1 : 1 or 1 : 2 in the MedSurg ICU.
If no vents or drips are involved, the ratio may be 1 : 3.
The unit accommodates a total of 42 beds.
Required certifications include BLS, ACLS, and NIHSS.
Common diagnoses and types of patients include :
Cardiac cases such as open heart surgery, balloon procedures, and impella placements.
Patients receiving tPA or TPE.
Level 3 trauma cases are treated on this unit.
Experience with trauma is not necessary, as core staff handle such cases.
Essential skills include vent management, sedation, sepsis, pressors, DKA, cardiac issues, CHF, and respiratory challenges.
RNs are required to titrate drips.
Common titratable and set rate drips include :
Vasopressors such as Epi, Levophed, Phenylephrine, and Vasopressin.
Inotropes like dopamine, dobutamine, and primacor.
Antiarrhythmics including amiodarone, cardizem, esmolol, lidocaine, and procainamide.
Anti-hypertensives such as Clevidipine, labetalol, cardene, Nipride, and nitro.
Beta adrenergic medications like Isuprel.
Sedatives including ketamine, Precedex, propofol, and Versed.
Paralytics like vecuronium, Nimbex, and rocuronium.
Analgesics including fentanyl and morphine.
Anticoagulants such as Aggrastat, tPA, Argatroban, Heparin, and Integrilin.
Insulin is also utilized.
A charge nurse is not present on each shift, as staff are involved in codes and condition help.
Patient care techs are available on this unit.
Ratios for patient care techs typically stand at 1 : 10.
Patient care techs assist with EKGs, blood sugar monitoring, setup of procedures, I / O, ADLs, and lab draws.
A hospitalist or intensivist is available in-house 24 / 7.
During the day, there are two MDs and one NP.
At night, one NP and one MD are on duty.
Respiratory therapy is available in the ICU 24 / 7.
Respiratory therapists handle vent settings, intubation, and ABGs.
They also assist physicians with bedside tracheostomies.
Unit clerks and monitoring techs are part of the support team.
Centralized telemonitoring occurs in each nurse's station, as well as in patient rooms.
Shifts are scheduled from 6 : 45 PM to 7 : 15 AM, with a grace period for clocking in and out.
Staff must clock out for lunch.
Self-scheduling is available, with four requested days off in a four-week period.
Weekend requirements include four weekend shift days in four weeks.
Two major holidays are required, with only Christmas's holiday observed if applicable.
There are no call or standby requirements.
Floating to other units is required but limits the patient ratio to four patients instead of six.
Scrub color is navy blue.
The cancellation policy allows cancellation of hours equivalent to the guaranteed assignment hours.
Charting is conducted using the Epic system.
A BSN is not required.
This position involves working in an acute care setting.
