Job descriptionJob Description : Key Responsibilities : Develop and implement individualized care plans based on client needs, goals, and preferences. Coordinate with healthcare providers, caregivers, and other professionals to ensure seamless delivery of services. Monitor and evaluate client progress, making adjustments to care plans as necessary. Serve as a liaison between clients, families, and service providers to facilitate communication and address concerns. Advocate for clients to ensure they receive appropriate services and resources. Maintain accurate and detailed records of client care and interactions. Stay informed about industry regulations and best practices to ensure compliance and continuous improvement. Qualifications : Relevant certification or licensure as appropriate to the discipline. Proven experience in care management, case management, or a similar role. Strong understanding of healthcare systems, regulations, and resources. Excellent interpersonal, communication, and organizational skills. Ability to work independently as well as part of a multidisciplinary team. Empathy, patience, and a commitment to providing person-centered care.
