Job Duties :

Conduct visits shortly at beginning of a patient's home health episode

• Perform intake assessments and support appropriate documentation for transitions
• Assess for clinical stability and determine if urgent or early primary care follow-up is needed
• Summarize recent care history and current clinical status
• Document clinical findings in the EMR to update care records and flag any clinical concerns
• Provide education and emotional support to patients and their families
