Description :

Monday to Friday schedule from 8 : 00 AM to 4 : 30 PM.

Occasional weekend work may be required.

Role involves working as a discharge planner for various departments including medical / surgical, tele, ICU, emergency department, and post-surgical care.

Only discharge planning social workers should apply.

Candidates must have recent experience in an acute in-hospital setting within the last 2 years.
