Certified Home Health Agency Registered Nurse (CHHA RN) Responsibilities :

• Conducts home visits providing skilled clinical assessments, patient education, and direct nursing care to their patients in their own homes.
• Coordinates skilled services within the patient’s interdisciplinary team as well as directs the patient’s paraprofessional services.
