Responsibilities
What You Will Do :
Work collaboratively with physicians, nurse practitioners, nurses, therapists, social workers, and caregivers to deliver comprehensive and coordinated care to our patients in their homes – conduct patient home visitsAssess, plan, direct, and evaluate each patient’s care plan to ensure it meets the patient’s needsManage emergency situations in a prompt, precise, and professional mannerComplete medication administration and treatment needs for all assigned patientsProvide nursing care in accordance with applicable regulations and Company policyPrepare and maintain accurate and complete clinical and progress notes on a timely basisQualifications
Qualifications
