Job descriptionProvide hands-on care, assessments, and treatment to older adults Collaborate with physicians, nurses, and an interdisciplinary team to develop and carry out care plans Order, interpret, and follow up on labs, diagnostics, and results Initiate emergency measures and coordinate patient care as needed Educate participants, families, and staff to support better health outcomes Document care accurately and follow program procedures Participate in quality improvement initiatives and care planning meetings Active Pennsylvania Nurse Practitioner or Physician Assistant license Experience working with older adults in a clinical setting or an interest in developing this skill through provided training CPR and First Aid certification Valid driver's license and ability to travel for participant visits and training Strong communication skills and the ability to work independently and as part of a team This pay range represents Covelo Direct's good faith minimum and maximum range for this role at the time of posting. Actual compensation offered will be dependent on a variety of factors, including, but not limited to, the candidate's experience, qualifications, and location.
