JOB DETAILS Practice Setting : Hospital based Reason for Coverage : Provider vacancy Dates of Coverage : Coverage will be minimal What Percentage (%) of the Practice is Neonate, Pediatric, Adult, Geriatric : 100% adult and geriatric What Percent of the Practice is Outpatient, Inpatient : 100% inpatient Average Number of Office / Clinic Patients per Day : n / a Will Other Urologists be Present During Locum's Assignment : yes How Many Urologists in Your Group : 5 Lab Onsite : Yes X-Ray Onsite : Yes Describe Office Ancillary Staff : NA Trauma Level / Designation : Level 3 Required Areas and Procedures : General Urology, Ureteroscopy If travel is approved, do you have any preferences or additional caps not outlined in the travel policy : No Additional Notes : This service provides inpatient APC support 7 days a week Submission Requirements MUST NOTATE ALL DURING NAME CLEARS Board Certified / Board Eligible - REQUIRED Active SC license (will accept providers needing to obtain state license) - REQUIRED ACLS, BLS - REQUIRED DEA - REQUIRED MUST submit to both locations to be considered - REQUIRED MUST be available to provide coverage of up to 6 days per month - REQUIRED
