- Setting : HYBRID (Home Visits & Virtual)
- Location : San Antonio, TX
- Schedule : Mon-Fri (8a-5p)
- Duration : Permanent
- Duties :
- Core NP (Homecare) : Travel to patients homes
- Conducting assessments both in patients homes and in virtual environment
- Counsel and educate patients and families about benefits and programs available.
