Job descriptionBelieves in the power of integrated care and the connection between physical and mental health Enjoys working both one-on-one with clients and collaboratively with multidisciplinary teams Wants to help clients navigate the healthcare system and promote healthier lifestyles Thrives in a mission-driven, community-focused environment Coordinate care for up to 200 clients, completing health screenings and identifying physical health risks Support wellness planning by setting physical health goals and promoting healthy lifestyle changes Educate clients on chronic condition management, appropriate healthcare usage, and health prevention Complete screenings (e.g., metabolic syndrome), medication reconciliations, and health consultations Collaborate with PCPs, specialists, and community partners to ensure holistic, person-centered care Participate in interdisciplinary team meetings and provide nursing support across clinic locations as needed High School / GED required Practical Nursing or Associate of Nursing degree required Bachelor’s degree in nursing preferred Proficiency in Microsoft Office Suite and Electronic Medical Records preferred A valid driver's license and agency established minimum automobile coverage required Licensed Practical Nurse (LPN) required Registered Nurse (RN) preferred Are passionate about improving healthcare access for individuals with behavioral health needs Enjoy client education, outreach, and being a reliable support through their wellness journey Are confident in your ability to identify health risks and coordinate appropriate care Thrive in both independent work and team collaboration Are committed to culturally competent, strength-based care that meets people where they are Retention bonuses paid as per bonus policy; exclusions may apply.

Support Whole-Person Care as a Nurse Care Manager in Our Behavioral Health Healthcare Home
We’re seeking a Nurse Care Manager to join our integrated care team and make a meaningful impact on the health of individuals with behavioral health needs. In this role, you’ll coordinate physical health support for clients in our Healthcare Home program—bridging the gap between behavioral and medical care to help clients live healthier, fuller lives.
