About the Space :
Community Living Room (CLR) is a drop-in based resource linkage program, focused on connecting people who use drugs to substance use services and treatment, such as medically assisted treatment (suboxone and buprenorphine), health education support groups, and safe use supplies. The drop-in also provides coffee, breakfast and lunch and additionally, a case management program that focuses on two areas : medical and community case management services every afternoon.
About the Role :
The Case Manager is responsible for conducting intake and assessments, including follow-up assessments, and linkage to the necessary resources to seamlessly support clients in the Community Living Room. The Case Manager collaborates and works with other Case Managers, Community Health Workers, and Community Ambassadors within SFCHC programming and other organizations, such as SFDPH, to ensure case management services are provided during and after the program.
The Case Manager is also responsible for working with their assigned case load to develop care plans and coordinating participants through navigating medical care, health education, supportive counseling, benefits advocacy, and coordinating services within their program as assigned by their direct supervisor, the Community Living Room Program Manager. Direct client work and interaction will be done at the office at our main clinic at 730 Polk Street, or at our various satellite locations within San Francisco.
Essential Duties :
Conduct intake and assessment of clients accessing SFCHC's Community Living Room drop-in program, with special emphasis on supporting clients who use drugs.Provide supportive counseling and develop service goals with clients.Provide as needed health education and linkage support to primary and behavioral health care.Provide as needed harm reduction counseling to clients using substances, and linkage and navigation to substance use treatment and services within SFCHC and greater Bay Area.Conduct quality assurance on referrals to ensure that follow-up and referral completion is reached, if needed.Work collaboratively with the client's care teams to identify their needs and develop goals accordingly.Navigate and outreach clients as needed to support with care engagement.Facilitate and support educational workshops and groups, working collaboratively with the Behavioral Health team to ensure workshops and groups meet agency standards.Complete documentation, reporting, and treatment plans in a timely manner.Work collaboratively with direct supervisor to ensure enrolled participants receive comprehensive case management once they are enrolled in the program.Work collaboratively with staff within SFCHC programming, supporting clients accessing programs within various department drop-ins, if needed.Work collaboratively with SFCHC staff and assist as needed with field-based outreach.Attend staff meetings and trainings as required.Participate in, and comply with, agency quality assurance, peer review, and clinical consultation.Perform other duties and tasks as needed.Minimum Qualifications :
Must be able to sit, stand, and walk for extended periods of time.Preferred BA / BS in a health sciences or social work-related field or AAA / CMH / CHW OR comparable work experience.One year of experience providing case management, counseling, advocacy, health education, service coordination, and / or outreach work to disenfranchised communities.At least one year of experience working, or interpersonal experience with transgender individuals and issuesExperience working with diverse populations including BIPOC, the LGBTQ+ community, refugees / immigrants, people living with HIV / AIDS, people experiencing homelessness, and multiple diagnosed populations (substance use and / or behavioral health issues).Knowledge of harm reduction philosophy, client centered counseling, and motivational interviewing techniques and trauma informed care.Must be adaptive, flexible and a team player.Strong verbal and written proficiency in English.Demonstrated excellence in attendance and reliability, interpersonal, organizational, verbal, and written communication skills.Familiarity and experience with Microsoft Word and Outlook programsDetail-oriented with strong follow-through skills.Up to date with vaccinations, including the latest COVID-19 and seasonal flu shot vaccines.Proof of TB clearance (issued within the past 12 months).Spanish speaking is highly preferred.Benefits :
