About the role :

Full-time, permanent position

Monday-Friday 8a-5p

AGNP / PA will work in an outpatient setting seeing an underserved adult / geriatric patient population

Adult Geriatric Nurse Practitioner / Physician Assistant will be responsible for Primary Care services helping individuals manage their chronic conditions, preventative care, and patient education

APP will join a busy practice that offers a collaborative, team environment

Background :

Board Certified Adult Geriatric Nurse Practitioner / AGNP or Physician Assistant / PA

2 years of experience as an Adult Geriatric Nurse Practitioner or Physician Assistant required
