Practice setting : Outpatient primary care clinic

Types of procedures : Preventive health exams, wellness visits, acute episodic care, chronic disease management, patient education, ordering and interpreting diagnostic testing, prescribing within scope

Ages : Adults and children depending on site

Requirements : Background check within thirty days prior to hire (Wisconsin placements require Wisconsin background check), drug screen within thirty days (ten panel), BLS certification from American Heart Association, American Red Cross or ProCPR blended with hands on portion, Hepatitis B, TB, application review, active license to practice, five year work history with explanation of any gaps greater than ninety days, current and previous five year hospital privileges, DEA, CDS registration, board certification, medical education verified by primary source, malpractice certificate of insurance, license sanctions review, Medicare and Medicaid sanctions review, federal program exclusions check, malpractice history, NPDB self query

Additional information : All credentialing documents must be provided and verified prior to assignment. Compliance with federal and state regulations required
