Reports to : Care Navigator Manager- Mental Health Team

Employment Type : Full-Time. Hybrid (Remote & In-Person at Old Fourth Ward, Atlanta, GA), Must reside in Georgia. Monday, Tuesday, Thursday, 7 : 30am-6pm.

Position Summary

Step into a pivotal role as a Care Navigator on our Psychiatry and Mental Health team, where your contributions will directly support both providers and patients with compassion and efficiency. As a Care Navigator, you'll serve as a key patient support liaison, often becoming the first point of contact for new members. In this role, you'll showcase Ascend's commitment to patient-first, accessible healthcare.

You'll leverage your expertise to handle administrative tasks, coordinate patient care, and facilitate seamless communication in both virtual and in-person care settings.
