Physical Therapist (Home Health), Full Time Days

The Physical Therapist evaluates patients, establishes functional goals and treatment programs, treats patients according to diagnosis utilizing therapeutic modalities and procedures as prescribed, supervises and delegates tasks to supportive personnel, maintains appropriate records using departmental documentation (EHR), communicates patients progress to physicians and other health care workers as indicated, meets department productivity expectations, accepts responsibility for personal and professional growth and performs other duties as assigned. This position must be able to demonstrate the knowledge and skills necessary to provide care and service appropriate to the age of the patients served in his / her area.

Pay Range : $29.96 - $49.25 / hour

Employment Type : Full Time, 72 hours every 2 weeks

Requirements : Bachelor's Degree in Physical Therapy required, Michigan Physical Therapist License required, BLS for the Healthcare Provider within 60 days of hire required

Clinical Evaluation and Treatment

• Chooses appropriate and up to date evaluation tests and tools relevant to diagnosis and reason for referral.
• Develops goals and treatment plans appropriately.
• Demonstrates knowledge of, and effective use of, appropriate therapeutic techniques throughout the course of treatment.

Michigan Physical Therapist License required
